When you’re pregnant after loss, it’s important to know your provider understands your situation. Whether you have special medical considerations in this pregnancy or even if you “just” need emotional acknowledgement of the extra worries and fear, it’s a valid need.

These FREE printable cards are an ideal way to communicate with your provider that your pregnancy is unique. Download and print, then attach to your intake form at each appointment, or ask your provider to place on the outside of your medical records.

Cards are sized to print 4.25″ wide x 5.5″ tall. You can print them singly, or download the template for the Avery 5689 postcards that print 4 to a page.
